Adoption Questionnaire

Thank you for filling out this questionnaire. Your answers will help us match your needs with the dogs in our program. Adoption fees are generally $350 for dogs 12 months or younger and $300 for dogs over 12 months.

Due to the large volume of questionnaires we receive we are only able to process those which have been completely filled out. Please be aware that we are an all volunteer group, most who work at full time jobs. It will generally take us 4-5 days to contact you after receiving your questionnaire.

If you would prefer to print, fill out, and mail or fax the form to us, please download our PDF version.  Mailing or faxing your form will likely delay the processing of your form by 5-7 days due to the need to process it by hand; we highly encourage you to fill out our online version.
